POSITION:                     RN Field Nurse

 

ORGANIZATION:          Jordan Health Services – Medicare

 

REPORTS TO:              RN Clinical Team Manager or Administrator

 

POSITION SUMMARY:

 

The RN Field Nurse is responsible for the timely performance, supervision, training, and documentation of clinical service delivery to the patient and his/her caregiver in accordance with accepted standards of nursing practice and regulatory guidelines.  S/he assists the RN Clinical Team Manager in improving the clinical and financial performance of the location.

DUTIES, RESPONSIBILITIES & COMPONENT PROPORTIONS:

 

I.            Work Flow Necessary to Ensure Clinical & Financial Success                                 (component proportion   75%)

 

  • Reports availability routinely, preferably weekly, to the scheduler.
  • Embraces and adheres to the use of electronic documentation via the organization’s software solution.
  • Completes assigned Work Flow tasks within timeframes allotted by the Division.
  • Responds timely to patient assignments and also responds in accordance with Divisional guidelines when s/he will not be able to accept assignments.
  • Performs and documents nursing service delivery related to patient admission and recertification including the OASIS data set and Plan of Care.
  • Completes OASIS data set information, as assigned, regarding patient events, i.e. Transfer to In-Patient Facility, Death In Home, Resumption of Care, Discharge, and Follow-Up.
  • Obtains and documents physician orders, when appropriate.
  • Responsible for the submission of all documentation and receipt of physician orders to ensure timely locking of OASIS and timely submission of billing.
  • Submits nursing orders and documentation, when appropriate.
  • Reviews data regarding patient assignments including but not limited to OAISS, Plan of Care, and Coordination Notes prior to service delivery.
  • Submits request for new supply requisitions when a new supply order is needed.
  • Responds to requests from location staff to clarify information needed for the Plan of Care (485) or OASIS.
  • Documents OASIS changes in the Coordination notes, when applicable.
  • Submits request to Add New Facility Event, as appropriate.
  • Assists with intake of patient referrals and in coordination of services between disciplines and other healthcare providers.
  • Participates in the reporting of lab results and/or abnormal findings to the appropriate healthcare professional, physician, and/or field clinician.
  • Provides supervision of home health aides and LVN/LPN team members, as assigned.
  • Participates in patient case conferences and documents ongoing Coordination of Care with other members of the healthcare team.

 

II.           QAPI Activities/Home Health Compare Reports                                                        (component proportion   10%)

 

  • Reports and documents Client Complaint and Client Occurrence Events, when appropriate.
  • Participates in the QAPI process of the agency, seeking to identify opportunities for performance improvement.
  • Reports significant patient findings to the RN Case Manager and the RN Clinical Team Manager and collaborates on actions or interventions to be taken.
  • Reports and documents patient infections per policy.

 

 

 

III.          Program Administration Coordination                                                                         (component proportion   15%)

 

  • Responsible for the appropriate use of agency resources including, but not limited to, supplies and personnel to ensure financial success.
  • Shares on-call responsibilities, as scheduled.
  • Works to maintain positive, collaborative relationships with patients, caregivers, referral sources, and/or physicians.
  • Maintains availability at all times during his/her scheduled hours of work, either in person or by telecommunications.
  • Maintains current knowledge of nursing practice and is responsible for personal continuing education required by the BNE.
  • Participates in internal staff meetings and training activities, as assigned.
  • Collaborates with members of the marketing team to assist in identifying potential referral sources, and patients appropriate for home care.
  • Performs all other duties as assigned.

 

 

JOB REQUIREMENTS:

Education

 

In accordance with licensing regulations, must meet the following criteria:

Must be a Registered Nurse (RN) licensed in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).

 

  • Experience
  • Minimum of two (2) years of active experience as a RN, preferred.
  • Possession of a working knowledge of Medicare/Medicaid home health care benefits, policies and procedures preferred.
  • Understanding of Medical Diagnosis Coding, preferred.

 

  • Skills & Abilities

 

  • Has a positive attitude and willingness to learn throughout employment.
  • Self-starter with excellent verbal and written communication skills.
  • Ability to work with both clinical and non-clinical staff members.
  • Willingness to learn and utilize software solution, inclusive of Divisional Work Flow Tasks.
  • Must be organized and have the ability to manage multiple tasks & priorities at any given time.
  • Possesses a firm foundation of basic nursing skills.
  • Willingness to learn home health regulatory standards related to job-specific duties, including but not limited to: OASIS, Condition of Participation, state-specific licensing rules, diagnosis coding, and internal policies.

A.   Must have satisfactory references from previous employers.

B.   Must be able to pass a background check which may include criminal history check and OIG sanction screening.

C.   Must be willing to submit to and be able to pass a random drug screening.

D.   Must have a dependable vehicle, a valid driver’s license, and maintain vehicular insurance in accordance with laws of the State.
